The office will be closing after July 30th coinciding with Dr. Kohles's upcoming retirement due to health issues; new patients are no longer being accepted. In-person appointments have limited availability.
Please send all record requests via fax to (855) 455-3240 after that date. The phone line will no longer be in service upon the office's closure.
Dr. Kohles will be hosting a retirement open house in the office on July 4th from 12-4pm.
We take most major insurances including almost all Medicare and Medicaid plans, though we are unfortunately no longer contracted with CareSource.
